About the role
Mercy Community are on the search to find an experienced Clinical Care Partner to join our Community Aged Care Services team based in Rockhampton on a Permanent Full-time basis.
The Clinical Care Partner is the designated person with clinical oversight under Community Care, responsible for delivering clinical care management and oversight that enables older people to live safely and independently at home. Working within a rights-based, person-centred framework, the Clinical Care Partner collaborates with participants, their supporters, and interdisciplinary teams to coordinate tailored, responsive and inclusive support
Salary commensurate with experience plus superannuation plus salary packaging available
This role involves but not limited to -
- Collaborate with participants to develop individualised care plans within required funding timeframes
- Incorporate the participant’s goals, service preferences, cultural needs, risk factors, and environmental requirements within the participant care plan and delivery
- Establish and update care plan and funding requirements, ensuring participants understand their available services
- Work as part of a team-based model, providing clinical care management and guidance to multidisciplinary team
About you
As the successful applicant, you will possess -
- Bachelor’s degree in nursing, Social Worker, Allied Health Professional or similar
- Demonstrated experience in care coordination, case management or similar roles in aged care, health, or disability services
- Solid understanding of the Support at Home Program, Quality Standards, and aged care rights
- Competence in interpreting clinical guidance and escalating appropriately
