About the role

Mercy Community Disability Service team have an opportunity for a passionate NDIS Support Coordinator. This is a Permanent Full-Time position located in Gladstone and provides support to participants within the Gladstone region.

 

A Support Coordinator works directly with people living with disability and their families, to coordinate, manage and maximise their NDIS plan.  You will truly make a difference in helping to enable greater independence across all domains of an individual's life.

 

$85,126 - $91,588 annually (SCHADS L4) plus superannuation plus generous salary packaging available

 

What you’ll be doing

As a Support Coordinator, your key duties will include:

  • Support participants to understand and create their NDIS plan, goals and funding access
  • Plan and coordinate participants to access support services in accordance with their allocated NDIS funds/budgets
  • Connect, establish and maintain participants with support services to pursue/achieve their goals
  • Use NDIS systems and processes to achieve the most of NDIS plans for the participant
  • Monitor, support, track and review participants NDIS plans/goals
  • Complete and provide reports to the NDIA with outcomes and success indicators relating to the participants plan/goals

 

About you

To be successful in the role, you will possess: 

  • Experience as a NDIS Support Coordinator (highly desired)
  • Hold a current Queensland Blue Card (or willing to apply)
  • Hold a current NDIS Workers Screening Clearance (or willing to apply)
  • Hold a current QLD Drivers Licence and have your own vehicle (mandatory)
