About the role

Mercy Community - Aged Care Services are on the lookout for a Care Manager (Full Time Permanent) to join our team in Rockhampton. Pay levels are determined by your experience and discussed during the recruitment process. 

 

This position is responsible for supporting the Facility Manager in the overall accountability and responsibility for the clinical management of health and clinical care outcomes of the residents in the facility. The role needs to ensure all residents are treated with dignity and respect and supports residents to exercise choice and independence. 

 

Your role involves but not limited to: 

  • Provide supervision to clinical staff to ensure evidence-based practice is implemented
  • Oversee the implementation of person-centred care and provide leadership to the team
  • Document and report on clinical and service outcomes
  • Oversee and ensure that infection control principles and procedures are maintained
  • Facilitate communication between staff and resident/representative to support partnering in care
  • Maintain working relationships with all care staff, medical practitioners and allied health professionals, ensuring documentation supports and optimises the Aged Care Funding Instrument (AN-ACC)

 

About you

To be successful in the role, you will possess:

  • Registered Nurse with AHPRA registration
  • Prior experience in aged care or an ability to demonstrate relevant experience
  • Experience in leadership roles or opportunities/skills
  • Ability to assess, plan, deliver and evaluate care for residents
  • Current valid Drivers Licence
