Mercy Community Services are searching for a Home Care Coordinator (Queensland Community Support Scheme - QCSS) to build and grow our Townsville services. This exciting opportunity will allow a unique individual to build and grow our Mercy Community Home Care services in Townsville.

QCSS is a service for those individuals, while not eligible for NDIS, experience disabilities, chronic illnesses, mental health conditions, or other circumstances that hinder their ability to live independently within the community. As a Home Care Coordinator, you’ll partner with clients & their families to support & ensure that their needs are met.

About your role:

Typically the Home Care Coordinator role will involve:

Lead and supervise a team of community support workers including completing performance reviews and identifying areas of development
Respond and assess referrals in the Service Provider portal
Liaise with external case managers, GP’s, and other allied health professionals for complex consumer needs
Monitor service delivery and provide statistical data
Negotiate, mediate, and resolve conflict as well as responding to emergency situations.

The unique part of this role, you will bring strategies, networking and marketing skills, to build a strong start for our team in Townsville.

About you:

Certificate IV in Aged Care Services or Community Support or the equivalent
At least three (3) years’ experience working within a Community setting in a similar role with leadership experience
Knowledge of community supports, inclusive of the Current Aged Care reforms
Hold a current QLD Drivers Licence and have your own vehicle (mandatory)
