Mercy Community Services are searching for a Home Care Coordinator (Queensland Community Support Scheme - QCSS) to build and grow our Townsville services. This exciting opportunity will allow a unique individual to build and grow our Mercy Community Home Care services in Townsville.
QCSS is a service for those individuals, while not eligible for NDIS, experience disabilities, chronic illnesses, mental health conditions, or other circumstances that hinder their ability to live independently within the community. As a Home Care Coordinator, you’ll partner with clients & their families to support & ensure that their needs are met.
 
About your role:
Typically the Home Care Coordinator role will involve:
  • Lead and supervise a team of community support workers including completing performance reviews and identifying areas of development
  • Respond and assess referrals in the Service Provider portal
  • Liaise with external case managers, GP’s, and other allied health professionals for complex consumer needs
  • Monitor service delivery and provide statistical data
  • Negotiate, mediate, and resolve conflict as well as responding to emergency situations.
The unique part of this role, you will bring strategies, networking and marketing skills, to build a strong start for our team in Townsville.
 
About you:
  • Certificate IV in Aged Care Services or Community Support or the equivalent
  • At least three (3) years’ experience working within a Community setting in a similar role with leadership experience
  • Knowledge of community supports, inclusive of the Current Aged Care reforms
  • Hold a current QLD Drivers Licence and have your own vehicle (mandatory)
