NDIS Support Coordinator - Logan / GC

About the role

Mercy Community Disability Service team have an opportunity for a passionate NDIS Support Coordinator. This is a Permanent Full-Time position located across both Logan and Gold Coast (Surfers Paradise). You will providing crucial support and assistance across teams, so if you are seeking variety in your work and office, this is the role for you.

A Support Coordinator works directly with people living with disability and their families, to coordinate, manage and maximise their NDIS plan.  You will truly make a difference in helping to enable greater independence across all domains of an individual's life.

$82,044 - $88,288 annually (SCHADS L4) plus superannuation plus generous salary packaging available

What you’ll be doing

As a Support Coordinator, your key duties will include:

• Support participants to understand and create their NDIS plan, goals and funding access
• Plan and coordinate participants to access support services in accordance with their allocated NDIS funds/budgets
• Connect, establish and maintain participants with support services to pursue/achieve their goals
• Use NDIS systems and processes to achieve the most of NDIS plans for the participant
• Monitor, support, track and review participants NDIS plans/goals
• Complete and provide reports to the NDIA with outcomes and success indicators relating to the participants plan/goals

About you

To be successful in the role, you will possess: 

• Experience as a NDIS Support Coordinator (highly desired)
• Hold a current Queensland Blue Card (or willing to apply)
• Hold a current NDIS Workers Screening Clearance (or willing to apply)
• Hold a current QLD Drivers Licence and have your own vehicle (mandatory)
