About the role
Mercy Community have an exciting opportunity for a full-time permanent, NDIS Support Coordinator role, to support NDIS participants based in the Rockhampton/Gladstone area. This is a floaters role between the 2 areas. A Support Coordinator works directly with people living with disability and their families, helping them maximise the funding that the NDIS has provided to the participant.
This role is paid at a Level 4 under the SCHADS Award, which equates to $82,044 + 11% Superannuation + Salary Packaging, per annum.
You will be able to deliver the following:
- Support participants to understand and create their NDIS plan, goals and funding access.
- Plan and coordinate participants to access support services in accordance with their allocated NDIS funds/budgets.
- Connect, establish and maintain participants with support services to pursue/achieve their goals.
- Use NDIS systems and processes to achieve the most of NDIS plans for the participant.
- Montior, support, track and review participants NDIS plans/goals.
- Complete and provide reports to the NDIA with outcomes and success indicators relating to the participants plan/goals.
About you
To be successful in the role, you will possess:
- Experience as a NDIS Support Coordinator (highly desired).
- Be committed to support the team within this floater's role across Rockhampton and Gladstone.
- Hold a current Queensland Blue Card (or willing to apply).
- Hold a current NDIS Workers Screening Clearance (or willing to apply).
- Hold a current QLD Drivers Licence and have your own vehicle (mandatory).
